Are you a compassionate and dedicated LPN looking to make a real difference in your community? Join Mains’l as a Licensed Practical Nurse (LPN) and become an essential part of our Health and Wellness Team.

As an LPN, you’ll provide wellness education, health support, and staff training under the direction of a Registered Nurse (RN) for individuals receiving community-based services. You’ll split your time between in-home visits, staff training, phone consultations, and collaboration with our team.

What You’ll Do:
  • Conduct home visits to collect health data and report changes to the RN
  • Train direct support staff on medical procedures and wellness practices
  • Consult with the RN and management team when health needs change
  • Participate in team meetings and work groups
  • Support new enrollments with significant medical needs
  • Help coordinate care by reviewing provider referrals

What We’re Looking For:

Our ideal LPN is:
  • Licensed and in good standing with the MN Board of Nursing
  • Experienced in home or community-based care (preferred)
  • strong communicator, skilled at simplifying medical information
  • Organized and able to manage time and multiple priorities
  • Compassionate, collaborative, and driven to improve the lives of others
  • Open to learning and committed to professional development

Salary: $25.36-$28.80/Hour
Schedule: Full-Time, Primarily Days, plus on-call rotation
